Known as Elamipretide, this tetrapeptide binds cardiolipin on the inner mitochondrial membrane to stabilize mitochondrial structure and support ATP production. It recently gained FDA accelerated approval, as Forzinity, for Barth syndrome, a rare mitochondrial genetic disorder.

SS-31 can cause a welt at the injection site. Bac water containing 0.9% sodium chloride reduces that reaction for some people. Plain bac water also works.
- Use bacteriostatic water as the diluent.
- Add the water slowly, letting it run down the inside wall of the vial rather than onto the powder.
- Swirl gently to mix, then leave it to dissolve. Do not shake.
- Give it a few minutes. The finished solution should be clear and colourless.

Standard protocol (subcutaneous)
- 5 mg to 10 mg per dose
- Two to three times weekly is the common frequency, though some run it daily
- At a 3 mL fill on the 50 mg vial (16.7 mg/mL), 5 mg is a 0.3 mL draw (30 units)
Dosing detail
- Common dose is 5 mg to 10 mg per injection, most often 2 to 3 times weekly (some dose daily).
- A mitochondria-targeted peptide (elamipretide), studied for energy, recovery and ageing. The pharmaceutical version was FDA-approved in 2025 (Forzinity) for a rare mitochondrial disease, at higher clinical doses than these.
- Effects are internal and gradual rather than something you see day to day.
Duration
- Run in 4 to 6 week blocks. It is used for cellular energy, recovery and ageing rather than a quick visible effect.
Notes

- A small welt can occur; saline bac water helps.
Storage and handling
- Keep the powdered (lyophilized) vial in a cool, dark place. Refrigerate it for longer-term storage before mixing.
- Once reconstituted and refrigerated at 2–8 °C away from light, use the vial within 90 days.
- Do not freeze a reconstituted vial, and keep it away from heat and sunlight.
- If the solution turns cloudy, develops floating particles, or shows any other sign of degradation, stop using that vial.
