Known as Elamipretide, this tetrapeptide binds cardiolipin on the inner mitochondrial membrane to stabilize mitochondrial structure and support ATP production. It recently gained FDA accelerated approval, as Forzinity, for Barth syndrome, a rare mitochondrial genetic disorder.

Mechanism
Its alternating aromatic-cationic sequence lets it concentrate heavily in mitochondria, where cardiolipin binding blocks damaging cytochrome c peroxidase activity and stabilizes cristae structure, improving electron transport chain efficiency, lowering reactive oxygen species, and preserving membrane potential and fusion/fission balance, especially during ischemia-reperfusion stress.
